Commit 994ccd11 contained a bug that caused new posts created via the
courseware interface to not get posted to the correct topic. They would
appear to work but would actually be posted to the "undefined" topic.
This has now been fixed.

Previously, buttons were activated on pressing Enter, but the expected
behavior is to activate on pressing Space. Several JavaScript errors
that prevented various buttons from properly activating via the keyboard
are also fixed.

The show/hide toggle now properly acts like a button, and focus is now
trapped on the loading element for both the show/hide action and loading
of a new page.

Now the interface will reset apropriately and allow a user to retry
expanding the discussion or loading a new page, and the alert message
will ask the user to retry.

This was merged too early--it's still missing the filtering and display-your-cohort changes.
Will need to revert this revert later.

Fix staff markers for inline threads and double staff markers for threads with deeply nested comments.

It worked in Chrome because Chrome appears to magically infer some missing
arguments to methods. This is the second time this happened, should be more
careful about it in the future, because it causes confusion when features
work for some students but not others.

Fix WMD editor reply boxes on embedded discussions, although I think the way it's implemented is something of a hack.

Needs different CSS to make it more appropriate in the embedded view.
