Updates to account for recent CSS changes in platform. Changes made include:
- Removing h3 element response__submission__status__title, this is more properly just a div
- Removing style %t-subheading, as %t-titlecase breaks a11y so %t-heading may be used everywhere
- Adding staff-assessment__display__header and staff__grade__title to _shame.css
- Modifying acceptance tests to account for loss of %t-titlecase

Fix leaderboard PDFs

PDF files cannot be displayed in an `<img>` tag, we should be using the pre-made template.
Also moving the image below the response because it looked weird above. Tests updated.

Version 1.0.0 release

Seeing as ora2 is reasonably stable at this point, having been in
production for some time, I move that we transition our versioning
scheme from "arbitrarily increment the last digit in 0.2.x" to
semantic versioning as recommended by GitHub.

This is arbitrarily being declared the 1.0.0 release, all subsequent
releases should follow the guidlines described at http://semver.org/

Fix for leaderboard image bug

Removing a "float:left" style to fix TNL-3115. Modified css files
are included.

Some tests were reliably failing on my shiny new "not-editably-installed"
ora sandbox, which led to some intense debugging.

Now finding/excluding files to allow ora to be installed in
non-editable mode, which will be done in edx-platform as part
of the next release.

Remove a submission from ORA by cancelling it to remove from grading
pools, then passing through to submissions.

Profiling changes and data

Made a few small optimizations to acceptance tests, added ability to profile
tests with an environment var, and updated .gitignore to ignore files
generated when running acceptance tests locally.

Update to latest nose, bok-choy.
